COMPLETING THE OPEN ENROLLMENT PROCESS IS MANDATORY FOR ALL

ACTIVE & RETIRED EMPLOYEES OF THE CITY OF HAZLETON.

The City of Hazleton is committed to providing quality benefits for all employees and retirees. This is the time to enroll or make changes to your medical, dental, and vision benefits if you wish. You may also add or remove coverage for eligible dependents (spouse and dependent children up to age 26). This is the one time of year when you can make changes to your benefits without experiencing a Qualifying Life Event (marriage, divorce, birth/adoption of a child, etc.). The benefit selections you make during open enrollment become effective on January 1, 2020. 

Benefits Open Enrollment Ends Nov. 15th at noon

No enrollments or changes can be made after this date unless you have a qualifying event.

The City of Hazleton will be holding Open Enrollment Assistance Workshops on the following dates:

Wednesday, November 6th, 1:00 – 4:00 p.m.

Thursday, November 7th, 3:00 – 6:00 p.m.

Wednesday, November 13th, 9:00 a.m. - Noon

Please attend one of these workshops for one-on-one assistance with this year’s open enrollment.

If you cannot attend one of these workshops, you will need to logon to the City’s enrollment portal, BerniePortal, and complete your elections.

What types of changes and additions should I make in the Portal?

  1. Name and address changes
  2. Dependent updates (add and remove)
    1. Please note: If adding a dependent, have their birthdate and SSN at time of enrollment, unless adding a newborn, then use all zeros in place of the SSN.
  3. New for 2020 -Active Employees Only! You can now elect voluntary life insurance coverage for you and your family. This is separate from the group life insurance that the City already has in place for employees and retirees. Voluntary Life Insurance is 100% employee paid.
  4. New for 2020 – Active Employees Only! You can now elect voluntary long-term disability insurance coverage for yourself. This insurance is 100% employee paid.

How do I access my account in BerniePortal?

A BerniePortal account was sent up for you during last year’s open enrollment or if you were newly hired in 2019 an account was setup for you at that time. Go to:

  1. https://app.bernieportal.com/en/login
  2. If you forgot your password, enter your username and select “forgot password”
  3. Complete the Personal and Contact Information sections
    1. Update and changes
    2. Press “Save and continue” to move through the sections.

After you complete the information section, Select “Begin Enrollment” button.

Guided Enrollment: Step 1

  • Please answer the questions regarding AFLAC. These questions require an answer, or you cannot move through the remaining steps of the guided enrollment.
  • If needed, you can download a copy of your most recent elections.
  • Press “Save and Continue” to begin your open enrollment elections.

Guided Enrollment: Step 2

  • Confirm that the correct dependents are listed
  • Add any missing dependents that you wish to enroll
  • I you wish to remove a dependent, please leave them listed on this page. You will remove them from the plan at a different part of the enrollment process.

Guided Enrollment: Step 3

  • Select your medical plan (unless waiving coverage – in this case select waive coverage)
  • Then select the coverage tier

Notes about dependent removal:

  • If you change your coverage tier to a lower level (ex. from Employee + Spouse to Employee only). the carrier will be notified to drop the dependent
  • In the case where family coverage needs to be selected but a dependent does not qualify, you can unselect the dependent by unchecking the box net to his or her name.

Guided Enrollment: Step 4

  • Check each box to finalize your enrollment
  • Electronically sign your name (initials are fine)
  • Press “I agree”
